Is this about physical healing or spiritual healing?

Matthew understood that as both. It does not mean that we can insist on it every time that we are sick. There is no guarantee that believers will always be physically healed by his stripes. There is 100% guarantee that by his stripes, our relationship with God is always restored if we believe. 1 Peter 2:

24 who Himself bore our sins in His own body on the tree, that we, having died to sins, might live for righteousness—by whose stripes you were healed
i.e., spiritually. Physically, Paul wrote 1 Timothy 5:

23 Stop drinking only water, and use a little wine because of your stomach and your frequent illnesses.
By his stripes, we are healed. Really?

Spiritually, always. Physically, sometimes. In any case, have faith in God.

I would offer God does not heal by the hands of dying mankind

The Holy Father using the temporal thing seen to give us his understanding

He used heel in Genesis 3 and Isiah 53 to represent the crushing of the head of the serpent bruising the heel of the Son of man ,Jesus

We have the new born again spirit. . all die not receiving the new incorruptible body.

A good example John the Baptist in jail waiting out his death sentence . Having doubt to whether it was the Christ working in the Son of man he sent two to represent the work of one.

Using the temporal literal healing to show the work of the gospel

I would offer . Its where I think much confusion comes in .

Most people look to the bloody demonstration and not the other two, as all three demonstrations working as one witness God spoke

The three day and night promised demonstration a parable of the Lamb that was slain from the foundation when the Father as Christ did work (6 days)

Three separate demonstrations beginning sufferings unto death. . the Father striking him in the garden of Gethsemane (the non bloody demonstration). The work of two the Father working with the Son of man Jesus. The promised witness of two (Genesis 3 and Isiah 53 ) fulfilling that prophecy

Then they moved to the hill (skull) the bloody carnal demonstration to the whole world giving the illusion that it was a Satan inspired mob that did the work which is accredited to the Father yoked with the Son (the fulfilled prophecy of two working as one. . .salvation)

It would seem in that way a sign and wonder after to the world in a hope of drawing mankind to His word as it is written A Faithful God who hides himself.
